Seguimiento farmacoterapéutico y uso racional de inhibidores de la bomba de protones en pacientes pediátricos hematológicos: una revisión sistemática

Resumen

El uso de inhibidores de la bomba de protones (IBP) es frecuente en la población pediátrica y se han descrito prácticas de prescripción potencialmente inapropiadas, especialmente en pacientes hospitalizados. En hematología pediátrica, la polifarmacia y la complejidad terapéutica hacen necesario evaluar periódicamente la necesidad, efectividad y seguridad de estos medicamentos. El objetivo de esta revisión sistemática fue analizar la evidencia publicada entre 2020 y 2025 sobre el seguimiento farmacoterapéutico y el uso racional de IBP en población pediátrica, con énfasis en su aplicabilidad a pacientes hematológicos. Se realizó una búsqueda y reconstrucción bibliográfica dirigida en PubMed/MEDLINE, Sco-pus, Web of Science, SciELO y Cochrane Library, siguiendo PRISMA 2020. La versión preliminar del manuscrito reportaba 642 registros, de los cuales 32 publicaciones fueron finalmente sintetizadas; durante la presente revisión editorial se verificó y caracterizó individualmente ese conjunto de 32 publicaciones. La evidencia directamente específica de hematología pediátrica fue limitada, por lo que se incorporó evidencia pediátrica general como evidencia indirecta y se diferenció expresamente de los antecedentes locales.  Los hallazgos muestran uso frecuente de IBP, prescripciones potencialmente innecesarias o prolongadas y señales de seguridad relacionadas con infecciones, microbiota y otros desenlaces.
