en el momento del drenaje de la infección, tan-
to cervicales como mediastinales, para identi-
car los agentes patógenos involucrados y propor-
cionar un esquema antibiótico dirigido.
Por otro lado el manejo de la vía aérea en este
tipo de pacientes es crucial, la intubación oro-
traqueal es difícil y cuando es fallida se pone
en peligro la vida del enfermo y en ocasiones se
necesita efectuar traqueotomía de urgencia.
Las complicaciones más frecuentes son sepsis y
síndrome de insuciencia respiratoria del adul-
to, las cuales se presentan en algún momento
del curso de la infección en todos los pacientes,
(100%) seguidas de derrame pleural y desnutri-
ción (30%), empiema y falla orgánica múltiple
en un 60% y neumonía en un 20% según Pérez,
et al.Los factores de riesgo asociados con malos
resultados son: diabetes mellitus, alcoholismo,
mala higiene oral, desnutrición, corticoterapia,
así como tratamiento inadecuado con antimicro-
bianos de igual forma la mortalidad se relaciona
con el diagnóstico y el tratamiento temprano (6-
18 h: 28.5% de mortalidad, >18 h: 42% de mor-
talidad.
Conclusión
La mediastinitis es un proceso no muy frecuen-
te con alta mortalidad que los médicos debe-
mos de pensar en pacientes con afecciones de
la cavidad bucal debido a que el diagnóstico y
tratamiento oportuno permiten una evolución
benigna.
Se trae acotación en este caso en vista de que
representa un alto costo de recursos físicos y hu-
manos que podría eliminarse con una adecuada
medicina preventiva bucal para evitar esta le-
sión mortal.
Es más frecuente en países subdesarrollados por
la escasa higiene bucal.
Los gérmenes más frecuentemente encontra-
dos son Streptococcus viridans, Staphylococcus
aureus y Staphylococcus epidermidis, así como
anaerobios entre los que se encuentran Bacte-
roides, Streptococcus Beta Hemolítico y los ae-
robios Gram negativos como Proteus, Pseudomo-
nas y E. coli.
